At Imathome, individual care planning and
personalised care begins at the point of
referral.

Matching
On receipt of referral the home will work closely with
the placing local authority to ensure that information is
shred to allow both the home and the local authority to
assess if this is correct placement to support the needs
of the child or young person. In addition the home will
complete a thorough matching process to ensure that
the move will have minimal impact on the placing
child or young person as well as the children and
young people already residing at the home.
Care Planning
From the moment it has been agreed that the
placement is able to support the children or young
person’s needs the home will endeavour to begin to
compile personalised care plans induvial individual to
the needs of the child or young person. The home will
strive to ensure these are in place and will form the
foundation. Care planning is reviewed regularly
throughout placement with the needs and voice of
the child or young person at the centre of this
process.
Transition Planning
The team at blossom house will work closely with the placing authority allocated social worker as well as
other professionals, carers and the child parents to ensure a robust transition plan. The team will share information such as an age-appropriate children’s guide of the home so that the placing child or young person has accesses to information which they can read and learn about the home in their own time.

In addition it will be advocated that transition days are highlighted where the placing child or young person can meet the homes manager and a member of key member of the team to try to begin to establish trusting relationships and get to know each other before the move. -
In addition a home visit will be scheduled where the placing child or you g person will be invited to lunch or dinner to view to home and ger a feel of where they will be living, during this visit they will be introduced to other members of the team, have a tour of the home and view their bedroom. If there are children or young people already residing at the home, then this will give them an opportunity to meet them face to face.

In addition the home will advocate an activity day where the placing children or young person will be invited to join the team and any children and young people at the home to take part in a fun, filled activity to continue to build and establish relationships and support the ease of the transition process for them.
Move Day
During the Transition planning stage a transition day
will be scheduled. The team will strive to ensure the
transition is as child friendly and smooth as possible.
The team will work closely with other providers, social
worker and parents through the process and the day
and team member that the child or young person has
met through the transition process will be at the
home to ensure a friendly face and support.
